The forms which should be reviewed and completed prior to the first appointment are as follows:

• Acknowledgement of Receipt of Privacy Practices

• Email and text consent

• Telehealth Consent

In order to authorize communication between my practice and other individuals or agencies, please complete one or more copies of the Authorization for Release of Medical Information form.
